メニエール病様の経過を呈した錐体部巨大コレステリン肉芽腫の1例

A Case of Cholesterol Granuloma of the Petrous Apex Presented with a Clinical Course of Ménière's Disease

　はじめに

　メニエール病の病態生理は内リンパ水腫であるとされている^1）。内リンパ水腫の成因については，内リンパの分泌過剰によるという説^2）と，吸収障害によるという説^3）が提起されてきた。今回われわれは，錐体部に発生した巨大腫瘤により内リンパ嚢の正常構造が破壊され，内リンパ液の吸収が障害された結果，内リンパ水腫を生じメニエール病の臨床経過を呈したと考えられる症例を経験しこので報告する。

A 47-year-old woman developed intermittent vertigo, constant dizziness, left hearing loss and tinnitus 3 years previously.

CT revealed a tumor at the petrous apex and the posterior fossa, with calcified margins and no en-hancement. Otoneurological examinations showed severe sensorineural hearing loss, left vestibular dysfunction and a positive glycerol test suggesting endolymphatic hydrops.

Surgery revealed that tumor had invaded the temporal bone and destroyed the superior semicircu-lar canal. Pathological findings showed cholesterol crystals and giant cells but no epithelial component, and diagnosis of cholesterol granuloma was made.

Dysfunction of endolymph drainage caused endo-lymphatic hydrops and showed the symptoms like Ménière's disease.
