Chubby Puffer症候群の1例における術前と術後2週，9か月目の睡眠ポリグラフ—混合型，中枢型，閉塞型への変化

Pre-and Postoperative Polysomnographic Change in Chubby Puffer Syndrome:Report of a Case

　はじめに

　Chubby Puffer症候群は，肥満に起因するPickwick症候群と，口蓋扁桃やアデノイドの肥大に起因するTonsiladenoid hypertrophy症候群の中間的存在として位置づけられている疾患で，1977年にStoolら^1）によって報告された。本症は，肥満や心肺症状の程度が軽度で，肥満に先行して慢性の気道閉塞症状があるのが特徴で，睡眠障害を伴い，睡眠時無呼吸症候群の原因疾患の1つである。本症候群の術前術後の評価に睡眠ポリグラフを用いた報告があるが，術後継続的に調べた報告は極めて少ない。

　今回われわれは，口蓋扁桃肥大および肥満が原因で睡眠時無呼吸発作を認めたと思われるChubby Puffer症候群の1症例に対して，外科的治療前後に睡眠ポリグラフを施行し，術前が混合型，術後2週目に中枢型，術後9か月目に閉塞型へと変化したので，若干の考察を交えて報告する。

A 36-year-old female presented with upper air-way obstruction due to tonsillar hypertrophy obesity and sleep disturbance. Polysomnographicexamination was performed preoperatively and the results showed obstructive and mixed sleep apnea. Two weeks postoperatively, obstructive and mixed sleep apnea was disappeared, while the central pattern was remained. Then 9 months later, central sleep apnea was disappeared, while obstructive sleep apnea recurred.

The fact may suggest that the central sleep con-trol damage is reversible.
