頭蓋底再建例における合併症の検討

Complications after Skull Base Reconstruction

　はじめに

　鼻・副鼻腔と頭蓋腔を確実に遮断し得る再建手術手技の進歩などにより，従来手術不能とされてきた頭蓋底浸潤のある鼻・副鼻腔悪性腫瘍症例に対して，頭蓋底と顔面骨を一塊として摘出する手術^1〜6）が，当科でも脳神経外科の協力を得て行われている^7〜9）。今回は，頭蓋底再建症例の術後合併症を検討し，頭蓋底の骨再建の必要性の有無について考察を加えた。

We performed 17 skull base reconstructions between 1989 and 1995, and 10 cases were recon-structed with a bone graft, 7 cases reconstructed with only soft tissue.

A free rectus abdominis musculocutaneous flap was a versatile flap for reconstruction of large combined defects of the skull base and face.

As for postoperative complications, five patients needed reoperation for the removal of necrotic bone resulted by infection.
