過度の発声により生じたと考えられた頸部および縦隔気腫の1例

Cervical and Mediastinal Emphysema Related to Excessive Phonation

　はじめに

　頸部皮下気腫は日常の診療においては，強い咳嗽発作や気管切開術直後に遭遇する以外に比較的遭遇する頻度の少ない病態である。

　今回，われわれは過度の発声により生じたと思われるまれな頸部皮下気腫の1例を経験したので報告する。

A 16-year-old male presented with sore throat, hoarseness, swelling of the neck and dyspnea. Plainx-ray and CT scan of the neck and mediastinum revealed cervical and mediastinal emphysema. On CT scan and bronchofiberscopy, the origin of the emphysema was not demonstrated.The cause of the emphysema was thought to be an elevation of pres-sure in the lower respiratory tract due to excessive phonation.
