　緒言

　鼻中隔に発生する腫瘍は，良性腫瘍，悪性腫瘍とも比較的まれとされているが、鼻中隔に発生する悪性腫瘍のなかでは扁平上皮癌が比較的多く，腺癌はとくにまれである^1〜3）。また，発生部位としては鼻中隔前方に比較的多く，後方，とくに上咽頭に接する鼻中隔後端に発生した腫瘍の報告は少ない。鼻中隔後端に発生した腫瘍の診断，治療は必ずしも容易ではない。われわれは，早期に発見し，治療し得た鼻中隔後端に生じたPapil-lotubular adenocarcinomaの症例を経験したので，文献的考察とともに報告する。

A 63-year-old man presented with papillotubular adenocarcinoma of the posterior end of the nasal septum. The extent of invasion of the tumor was evaluated with MR scans. The tumor was success-fully excised by transmaxillary approach. Low grade adenocarcinomas of the nasal septum are relatively benign. However, they often recur locally. Therefore, the optimal management of the tumor is a surgical wide resection. Transmaxillary approach is one of alternatives for a surgical resection of the posterior end of the nasal septum.
