症候性三叉神経痛—4症例の経験から

Four Cases of Symptomatic Trigeminal Neuralgia

　はじめに

　三叉神経痛は，三叉神経知覚根部周辺の刺激によって生じる顔面三叉神経領域の疼痛であるが，頭部，顔面，眼，鼻咽腔，口腔，歯の疼痛として初診時に各科を受診し，とくに他科領域の疾患で，自覚症状や臨床症状に乏しい場合には，原因不明の三叉神経痛として長期間経過していることがある。今回われわれは，初診時に特発性三叉神経痛として診断され治療を受け，後にその原因疾患が明らかとなった篩骨洞嚢胞，硬膜外膿瘍を合併した前頭洞嚢胞および脳腫瘍2例の記4症例を経験したので報告する。

Ethmoid sinus pyocele, frontal sinus pyocele with frontal epidural abscess, and two brain tumors were reported as regards to symptomatic trigeminal neuralgia. There are two types of trigeminal neuralgia (TN): idiopathic TN and symptomatic TN. Recently, the cause of idiopathic TN is thought to be vascular compression of the intracranial trigeminal nerve. Symptomatic TN is caused by various diseases. Particular attention should be paid to the patient with an intracran-ial disease, such as a brain tumor, who corn-plains only of trigeminal neulagia.
