鼻腔・膀胱の悪性腫瘍を合併したWerner症候群の1例

A Case of Werner's Syndrome Associated with Bladder and Nasal Malignancies

　緒言

　Werner症候群は1904年ドイツの眼科医であるWerner^1）によって初めて報告された遺伝性疾患であり，その特徴的体型と早期老化の諸症状が特徴とされる。本症候群の患者の平均寿命はその早期老化のため40歳代とされ，この点では予後不良であるといえる。また本症候群には悪性腫瘍が高頻度に合併することは有名で，一層予後を不良としている。

　悪性腫瘍の合併例の報告は既に諸家によってなされており，特に悪性黒色腫の合併例の報告は比較的多い。今回われわれは鼻腔の悪性黒色腫を合併し，さらに以前に膀胱腫瘍の既往を持つ本症候群例を経験したので，若干の文献的考察を加え報告する。

A 45-year-old woman was diagnosed as having Werner's syndrome associated with bladder and nasal malignancies.

She has suffered from transitional cell carcinoma of the urinary bladder, and has treated with TUR at 38 years of age. She has complained of nasal obstruction with intermittent epistaxis. The left nasal cavity was completely occupied with a black mass, which originated from the inferior turbinate. The histopathological study showed malignant melanoma. She was given two courts of DTIC combined with ACNU, which were not effective. The tumor was removed through a lateral rhino-tomy. She has been uneventful for 5 months after the surgery.
