上咽頭アミロイドーシスの1症例

Nasopharyngeal Amyloidosis: Report of a Case

はじめに

　アミロイドーシスは，本来生体には存在しない糖蛋白を主とするアミロイド物質が，細胞間隙に沈着する原因不明の代謝性疾患であり，現在一般に用いられているCohenの分類によれば全身性アミロイドーシスと，限局性アミロイドーシスに大別される。全身性アミロイドーシスは，肝臓，腎臓などの重要臓器に多発し，極めて予後不良の疾患であるが，限局性アミロイドーシスは，頻度も少なく，ある特定の器官に限られ，再発を見ることも少ない比較的良性の疾患である。頭頸部領域におけるアミロイドーシスとしては，喉頭アミロイドーシスがよく知られているが，報告されているものは比較的少数であり，鼻腔・咽頭のアミロイドーシスはさらに少ない。今回，われわれは，上咽頭に限局した，腫瘤形成型アミロイドーシスを経験したので報告する。

Amyloidosis is rarely found in the nasopharynx. A 40-year-old male complaining of a hearing loss in the right ear and sore throat. A mass with yellow-red color and smooth surface was found in the nasopharynx. There were no other amy-loid deposites in other sites of the body. Histo-pathological study of the tumor showed amy-loidosis with H.E. stain and Congo red stain.

The tumor was removed by surgical excision with an adenotome. He had no recurrence in 1 year after the operation.
