口唇・口腔癌の集学的治療—過去17年間の臨床経験から

Combination Therapy of Lip and Oral Cancers

　はじめに

　頭頸部癌の中で，口唇・口腔癌は喉頭癌と並んで頻度が高く，その領域が構音・咀嚼・嚥下に関与しているため，治療に際して常にquality of lifeが問題となる。また，その治療成績は期待どおりに向上しているとは言いがたく，発生頻度が高いだけに依然として頭頸部外科医を悩ましている疾患である。しかし一方では，近年の癌治療法の進歩は著しく，その動向によっては，今後治療成績の向上が期待できる疾患であると考える。

　今回，三重大学耳鼻咽喉科において過去17年間に経験した口唇・口腔癌症例を検討し，集学的治療の問題について考察した。

One hundred and sixty six patients with lip and oral cancer treated at Mic University Hos-pital from 1975 to 1991 were reviewed, with special attention to prognosis and recurrence. Survival rate was analyzed according to the Kaplan-Meier method. The 5-year survival rate was 79.4%. 64.1%. 44.6%. and 36.1%, respec-tively Inc stage I, II, III and IV. Sixty three in these patients were treated with intra-arterial chemotherapy, and the survival rate of T3 group indicated the improvement of the total survival rate. Combined therapy with systemic chemo-therapy is necessary for the improvement of the survival rate for lip and oral cancer.
