Benign Lymphoepithelial Lesion of the Parotid Gland

はじめに

　日常診療において唾液腺の腫脹を主訴として来院する患者は比較的多くその原因としては急性，慢性炎症や良・悪性腫瘍によるものが多くを占めている。その他腫瘍性病変をきたす疾患として唾液腺benign lymphoepithelian lesionが挙げられるが，その病態は未だ不明である。本疾患はSjogren症候群やMikulicz病における唾液腺の組織学的所見と同様の所見を呈することから，同一の疾患とする意見もある。今回われわれは，65歳女性で左側耳下腺部腫脹を主訴とし，口内乾燥感，涙腺や顎下腺の腫脹，その他，自己免疫疾患などの全身症状を欠く1症例を経験したので若干の文献的考察を加え報告する。

A 65-year-old female complained of an enlarge-ment in the left parotid gland for two years prior to admission. There were no other symptoms such as Mikulicz's disease or Sjögren's syndrome. His-tologically the normal parotid glandular tissue was replaced by diffuse lymphocytic cell infiltration. Lymphoid follicles and epi-myoepithelial islands were present in the lymphoid tissue, but no neo-plastic change was found in the tissue. The histo-logical features of the present case was identical to that of Sjögren's syndrome.
