頸部原発の結節性筋膜炎の1症例

Nodular Fasciitis in the Neck: Report of a Case

緒言

　結節性筋膜炎（Nodular Fasciitis）は1955年Konwalerが最初に独立した疾患として報告して以来多数報告されている^1）。本症は筋膜より発生し腫瘍性の増殖を示すがfibromatosisの一型に分類される良性疾患である^2）。主に四肢，特に前腕に好発するが頭頸部領域における発生も報告されている。病理学的に幼若線維芽細胞の増殖，核分裂像，および周囲への浸潤像など悪性を疑わせる所見を呈するため，fibrosarcomaとの鑑別が問題となることがある^2）。臨床的にも急速な発育を示し，硬い腫瘤を触れることが多く悪性疾患との鑑別が必要である。

　今回われわれは後頸部に急速に発育する硬い腫瘤を認め，諸検査で悪性を疑われたが，全摘後病理学的検索により本疾患と判明した症例を経験したので画像，病理所見とともに文献的考察を加えて報告する。

Nodular fasciitis is a benign and pseudosar-comatous lesion which is commonly observed in the extremities. This lesion usually represents a subcutaneous hard mass and is often misdiag-nosed as a malignant tumor. A case of nodular fasciitis in the neck which is a rare site of the lesion was presented. A 49-year-old male com-plained of a rapidly growing hard mass in his right neck. The diagnosis was made by open biopsy and a mass was surgically removed. The clinical course and the results of examinations including X ray, CT, ultrasound and scintigram were reported with the discussion in the light of available literature.
