喉頭顆粒細胞腫の1例

Granular Cell Tumor of the Larynx: Report of a Case

はじめに

　顆粒細胞腫は全身のほとんど全ての臓器に発生する通常良性の経過をとる稀な腫瘍である。喉頭の顆粒細胞腫は欧米では150例余りの報告を見るが，本邦では稀で文献上4症例の報告を見るにすぎない^1〜4）。今回著者らは喉頭部分切除術を必要とした喉頭顆粒細胞腫の一症例を経験し，組織学的に詳細に検討したところ本腫瘍の起源について知見が得られたので報告する。

A 47-year-old woman with granular cell tumor of the left vocal fold was presented. She com-plained of hoarseness and stridor. The tumor extended to the lower margin of the cricoid cartilage inferiorly and to the posterior commis-sure posteriorly, and was removed through a laryngofissure approach. The new vocal fold was reconstructed using a pedicle skin flap from the neck. Postoperative respiratory and phonatory functions showed satisfactory results. Electron-microscopic examinations of the tumor revealed the existence of myelin-like figures in the cyto-plasm of the tumor cells, including typical lyso-some-like electron dense particles and angulate bodies. These findings support the theory of neural origin of the granular cell tumor.
