はじめに

　Sjögren症候群は，主に唾液腺と涙腺へのリンパ球浸潤と破壊を特徴とし，口腔内乾燥や眼球乾燥をきたす症候群であり，耳鼻咽喉科にとって比較的馴染み深い自己免疫疾患である。Sjögren症候群の患者は，非ホジキンリンパ腫，特に粘膜関連リンパ組織（mucosa-associated lymphoid tissue：MALT）リンパ腫の発症リスクが44倍高いといわれている^1）。今回，Sjögren症候群の患者の治療経過中に，耳下腺に発生したMALTリンパ腫を経験した。持続する耳下腺腫脹という局所所見と，特徴的なMRI検査の所見からMALTリンパ腫を疑い，病理組織検査により診断できた。私達耳鼻咽喉科医は留意すべきと考えられるので報告する。

　We report a case of a patient with MALT lymphoma who was diagnosed during treatment of Sjögren's syndrome. The patient was a woman in her 50s with a history of Sjögren's syndrome, who was admitted to our hospital with a complaint of persistent parotid gland enlargement. She was subsequently diagnosed with MALT lymphoma. It should be noted that patients with Sjögren's syndrome have a 44-fold higher incidence of MALT lymphoma. CT and MRI are useful for the diagnosis of MALT lymphoma.
