篩骨動脈を茎とする鼻中隔粘膜弁を用いた鼻中隔穿孔閉鎖術の1例

A case of nasal septal perforation closure using unilateral mucosal flap based on the anterior ethmoidal artery

はじめに

　鼻中隔穿孔は，鼻中隔矯正術や鼻粘膜焼灼術などの外科治療，六価クロムなどの薬物による中毒，多発血管炎性肉芽腫症，梅毒などの全身疾患にて生じる^1）。無症状の症例も多いが，穿孔の大きさと位置によっては穿孔部の痂皮形成，反復性鼻出血，笛様音などを生じ，患者のQOLを低下させる。これまでにさまざまな閉鎖法が報告されているが，再穿孔をきたし治療に難渋することも多い。今回われわれは，篩骨動脈を茎とする鼻中隔粘膜弁で鼻中隔穿孔を閉鎖した1例を報告する。

　A nasal septal perforation（NSP）is a commonly observed nasal disorder. Most NSPs are traumatic in nature and often present with symptoms such as crusting, nasal obstruction, recurrent bleeding, dryness, whistling noise when breathing, or pain. Various operations are available to treat NSPs；we successfully closed a NSP using a unilateral mucosal flap based on the anterior ethmoidal artery（Castelnuovo's flap）.
