頸部リンパ節生検術に際し，悪性リンパ腫を疑う術前データについて

Usefulness of clinical and bilological data obtained prior to cervical lymph node biopsy

はじめに

　頸部リンパ節腫脹は，耳鼻咽喉科の日常診療において遭遇する機会の多い症状の1つである。その多くが細菌感染やウイルス感染による炎症性疾患であり，抗菌薬の投与や消炎治療により軽快するものが多い。しかし悪性リンパ腫や悪性腫瘍のリンパ節転移などの悪性疾患も存在し，診断のために手術を行うべきか，経過観察をすべきか迷う症例にしばしば遭遇する。そこで，診断のためにリンパ節生検を行うかどうか判断するための指標を見出すことを目的とし，悪性リンパ腫の鑑別のため頸部リンパ節生検術を施行した85例について検討した。

　Lymph node enlargement is a common manifestation of a variety of inflammatory and malignant diseases, and it is often difficult to make a definitive diagnosis from this state without lymph node biopsy.

　Lymph node biopsies were performed in 85 patients with cervical lymph node enlargement during a 10 year period in our department. Forty-nine, twenty-seven and nine patients were eventually diagnosed as having a malignant lymphoma, inflammatory or benign tumor, and metastatic carcinoma, respectively. In the malignant lymphoma group, patients tended to be older and had higher soluble interleukin-2 receptor（sIL-2R）and lactate dehydrogenase（LDH）levels.
