頸部リンパ節結核18症例の臨床的検討

Eighteen cases of cervical tuberculous lymphadenitis: a clinical study

はじめに

　2017年に日本では新たに1万6789人の結核患者の届け出があり，10万人対の結核罹患率は13.3であった^1）。日本での結核登録率は過去数年間減少傾向が続いているが，欧米諸国と比較するといまだに高い状況である。頭頸部領域を含めた肺門部以外のリンパ節結核は，肺外結核のなかで胸膜炎の次に多く，2017年に日本では新たに779人の届け出があった^1）。当院の医療圏である神戸市の結核罹患率は19.7（10万対，2017年）と，政令指定都市のなかでは大阪市（10万対で32.4，2017年），北九州市（10万対で21.6，2017年）に次いで多い^1）。

　今回われわれは，当科で経験した頸部リンパ節結核18症例の臨床的検討を行ったので報告する。

　We retrospectively examined the records of 18 cases of cervical tuberculous（TB）lymphadenitis diagnosed in our department. Nine of 11 patients with lymph node abscess were diagnosed as having TB lymphadenitis by fine-needle aspiration, and the remaining 9 were finally diagnosed by lymph node biopsy. The diagnosis of TB lymphadenitis was based on detection of acid-fast bacilli, culture and TB-PCR（polymerase chain reaction）. The diagnosis rate using fine-needle aspiration（81.8％）was comparable to that of lymph node biopsy（87.5％）in this series. For cases of lymph node abscess suspected to be TB lymphadenitis, initial use of fine-needle aspiration is recommended.
