嚥下機能改善型声門下喉頭閉鎖術の検討

Functional subglottic laryngeal closure for dysphagia：FSLC

はじめに

　誤嚥防止術が必要とされる重度の嚥下機能障害患者の場合，嚥下機能改善術のみを行っても誤嚥の制御は困難であり，むしろ確実な誤嚥防止が優先されるべきであると考えられる。しかし，嚥下機能が残存している症例に関しては，術前に嚥下機能を評価したうえで，機能改善効果のある手技を選択し，誤嚥防止術に追加することは有用であると考えられる。

　現在当科では，U字皮弁とV-Loc^TM（コヴィディエンジャパン社）を用いた声門下喉頭閉鎖術^1）（subglottic laryngeal closure using U-shape flap and V-Loc^TM：SUV）を誤嚥防止術として行っているが，これに輪状咽頭筋起始部離断術（cricopharyngeal myotomy at the attachment part：CPMA）や舌骨下筋群切断術などの嚥下改善手技を必要に応じて組み合わせて行う術式，嚥下機能改善型声門下喉頭閉鎖術（functional subglottic laryngeal closure for dysphagia：FSLC）を開発した。これまで誤嚥防止術と同時に舌骨下筋群を選択し，切断する概念はみられなかったため，その実際とともに報告する。

　We reported the concept and technique for functional subglottic laryngeal closure for dysphagia（FSLC）which was the latest operation in our department.

　FSLC is a minimally invasive method with few complications and it is an aspiration prevention technique that is advantageous also for speech reconstruction and swallowing function improvement. Presentation of 5 cases implemented in our department.
