穿孔を伴わない口蓋小欠損創の口蓋粘骨膜弁による即時再建—口蓋粘表皮癌2症例

Immediate reconstruction of palatal non piercing defect using mucoperiosteal palatal flap：two cases of palatal mucoepidermal carcinoma

はじめに

　軟硬口蓋の小唾液腺腫瘍摘出術では，鼻咽腔側に軟組織が保存できると二次的治癒（secondary intention）^1）が期待できることから，欠損を修復せずに手術を終了することが一般的である。ほとんどの症例は合併症なく治癒するが，時に鼻咽腔口腔間瘻孔や鼻咽腔閉鎖不全などの合併症を生じることがあり，その後患者側・治療者側はともに新たな負担を強いられる^2,3）。筆者らは，鼻咽腔側に軟組織を保存できた症例であっても，合併症の発生が懸念される場合には，口蓋局所の粘骨膜弁による即時再建を行っている。今回，上記を施行した2症例について考察を加えて報告する。

　In palatal tumor operation, if soft tissue can be preserved, surgery is generally terminated without reconstruction of the defects because of the secondary intention is expected.

　Most of such patients cure without wound complications, but in a few patients fistula formation and velopharyngeal insufficiency may occur and a new burden arise between the patient and doctors. Authors are try to perform immediate reconstruction by palatal mucoperiosteal flap to prevent any wound complications even if they are wound in which thin-mucosal tissues is preserved. In two cases of mucoepidermal carcinoma on the midline of the boundary line between soft and hard palate are reported.
