ビスホスホネートの副作用と考えられた外耳道骨壊死の1例

Bisphosphonate-induced osteonecrois of the external ear canal：a case report

はじめに

　ビスホスホネート（bisphosphonate：BP）は悪性腫瘍の骨転移による高カルシウム血症などの抑制や，骨粗鬆症による高齢者の骨折の予防を目的とする薬剤として広く使用されている。その副作用として頭頸部領域では顎骨壊死が知られている。頭頸部領域における他の部位の骨壊死として，海外においては外耳道骨壊死の報告が散見されるが，本邦ではBPによる外耳道骨壊死の報告はまだみられない。

　今回，筆者らはBPによる外耳道骨壊死と考えられる症例を経験したので，若干の文献的考察を加えて報告する。

　We reported a rare case of osteonecrosis of the external auditory canal associated with oral bisphosphonate therapy. The patient was an 88-year-old woman with bisphosphonate therapy for treatment of osteoporosis for five years and seven months. Unilateral external auditory canal skin fell into necrosis, and the canal bone was exposed with several partial hollows. Bone destruction of the external ear canal was confirmed by CT scan. Bisphosphonate was discontinued immediately. The patient has been under application of aluminum acetate liquid, ointment and linear polarized infre-red ray phototherapy.
