The restoration of composite tissue defects of the mandible remains a difficult problem. Re-cently it has been reported that an osteocutaneous scapular flap is a useful composite flap for com-plex mandibular reconstruction, because the skin paddles have an independent vascular pedicle from the bone flap, thereby allowing three di-mensional positioning of them for reconstruction. However, this technique is not so prevalent in Japan.

A 62-year-old man underwent a right partial mandibulectomy owing to ameloblastoma of the lower jaw. A bilobed scapular-parascapular skin flap and a 9cm length of scapular bone based on the pedicle of the circumflex scapular vessels was used to reconstruct the mandible. Bone scan was done at 1 month postoperatively and it revealed that the transplanted bone was viable.
