小児両側同時性顔面神経麻痺の1例

Bilateral facial nerve palsy observed in a 2-year-old girl

はじめに

　両側性顔面神経麻痺は稀な疾患である。顔面神経麻痺の発症するタイミングにより，①一側麻痺後2週間以内に他側麻痺が発症する両側同時性麻痺，②先行する麻痺が回復したあと他側に麻痺が発症する両側交代性麻痺，③両側に3回以上の麻痺が生じる両側再発性麻痺の3種類に分類される^1）。今回，小児の両側同時性顔面神経麻痺症例を経験したので報告する。

　Bilateral facial nerve palsy is rare. We report a case of a 2-year-old girl who had bilateral facial nerve palsy. She initially had an isolated right side facial palsy. Two days later, she presented a mask-like face with the opposite side facial palsy. As a result of further examination（blood CSF, brain CT）, there was no evidence of systemic disease, therefore the diagnosis of bilateral Bell's palsy was made. She was treated with glucocorticoid at tapering dose, and the palsy improved in 19 days.
