外傷性両側性顔面神経麻痺の1例

Bilateral Traumatic Facial Nerve Palsies: Case Report

はじめに

　両側性顔面神経麻痺は，糖尿病，リウマチなどの膠原病，あるいはギランーバレ症候群，サルコイドーシスなどの全身性疾患の部分的な症状として発現することが多いが，頻度は末梢性顔面神経麻痺の5％以下と低い^10）。外傷に起因するものは，さらにまれである。

　われわれは，頭部外傷後，両側性顔面神経麻痺をきたした成人男子の1例を経験したが，受傷直後には意識障害や顔面の腫脹を伴い，さらには顔面神経麻痺が両側性であるために，顔面の運動障害が見落とされ易く，診断や治療が遅れることが考えられたので，その特徴や問題点について，文献的考察を加えて報告する。

A case of bilateral facial nerve palsies follow-ing closed head trauma is presented.

Based on analyses of 15 cases (including ours) of bilateral traumatic facial palsies in the litera-ture, it is emphasized that bilateral facial nerve palsies may easily be overlooked due to lack of facial asymmetry or severe swelling of the face in the acute stage of head injury, and that since functional prognosis of traumatic facial nerve palsy is poorer in bilateral cases than in unila-teral cases, early diagnosis is mandatory and early surgery should be considered particulary for the cases of bilateral facial palsies that occur-red immediately after injury.
