Cholesterol granuloma may erode into the middle ear,the mastoid bone and the petrous apex. However, cholesterol granuloma developed in the cranial cavity is rare. We presented a case of brain abscess caused by a large mastoid cholesterol granuloma extended into the cranial cavity. The patient was, complaining of headache and paralysis of the left arm. MRI showed a huge mass in the left temporal bone and a mass in the cerebellum. The huge mass was removed by surgery via mastoidectomy and middle cranial fossa approach. The patient had no neurological complications postoperatively.
