脳膿瘍をきたした側頭骨巨大コレステリン肉芽腫の1症例

Brain abscess caused by a large mastoid cholesterol granuloma extended into the cranial cavity

Ⅰ　はじめに

　コレステリン肉芽腫は，その発生部位より乳突腔を母地とする中耳コレステリン肉芽腫と錐体部に発生する錐体部コレステリン肉芽腫に大別される。中耳コレステリン肉芽腫はしばしば遭遇する疾患であるが，頭蓋内に進展することは稀である。今回われわれは，左乳突腔から発生し，頭蓋内に進展，2次的に脳膿瘍を形成した1症例を経験したので，若干の文献的考察を含めて報告する。

　Cholesterol granuloma may erode into the middle ear,the mastoid bone and the petrous apex. However,cholesterol granuloma developed in the cranial cavity is rare. We presented a case of brain abscess caused by a large mastoid cholesterol granuloma extended into the cranial cavity. The patient was,complaining of headache and paralysis of the left arm. MRI showed a huge mass in the left temporal bone and a mass in the cerebellum. The huge mass was removed by surgery via mastoidectomy and middle cranial fossa approach. The patient had no neurological complications postoperatively.
