涙囊原発移行上皮癌の1例

Transitional cell carcinoma of lacrimal sac

Ⅰ　はじめに

　頭頸部領域に発生する腫瘍の中で，涙囊原発腫瘍は比較的稀であり，現在までに国内外合わせて約400例が報告されている^1）。その70％が悪性であり，非常に悪性率の高い腫瘍である^1）。これらの報告は眼科・形成外科からによるものが多く，耳鼻咽喉科からの報告は少数である。しかし，涙囊悪性腫瘍で隣接する鼻涙管や鼻腔などへの進展を認める場合，涙囊だけではなく眼窩内容物および鼻・副鼻腔の構成骨を含めた上顎全摘出または部分切除などの拡大手術が必要となるため，耳鼻咽喉科・頭頸部外科・眼科・形成外科が協力して治療に当たる必要がある。今回，非常にまれな涙囊原発移行上皮癌症例を経験したため，若干の文献的考察を加えて報告する。

　A 68-year-old man complained of a swelling of his right inner cantus. Magnetic resonance imaging demonstrated a tumor in the right lacrimal sac,nasolacrimal duct and nasal cavity. The pre-operative diagnosis was possible malignant tumor. Medial maxillectomy and orbitotomy were performed and subsequently the orbital cavity was covered with his abdominal rectus muscle. Pathological diagnosis was transitional cell carcinoma,and the patient received radiation（60 Gy）. He has been tumor free for the following one year.
