前頭洞に発生した血瘤腫の1例

A case of organizing hematoma in the frontal sinus

Ⅰ　はじめに

　血瘤腫は1917年に田所^1）によって最初に報告された出血性血液や凝血塊を主体とした偽腫瘍の臨床的病名である。比較的稀な疾患であるが，臨床所見や画像所見から悪性腫瘍との鑑別を要す。副鼻腔では上顎洞の発生が多数を占め^2～15），前頭洞の発症例は稀である^16,17）。今回われわれは左前頭洞に発生した血瘤腫の1例を経験したので若干の考察を加えて報告する。

　A 81-year-old female visited our hospital with the chief complaint of a swelling in the forehead. CT and MRI showed an expansive mass in the frontal sinus with the bone defect. Histopathological examination revealed organizing hematoma. Although we recommended the drainage operation of the mass endonasally,the patient refused the surgical treatment because of the benign tumor,old age and pancytopenia. The tumor had been stable until now. It was suspected that this hematoma was mainly occurred by the local bleeding and the obstruction of nasofrontal duct after trauma.
