頭蓋外内頸動脈瘤の3症例

Three Cases of Extracranial Internal Carotid Aneurysm

　はじめに

　頭蓋外内頸動脈に発生する動脈瘤は稀な疾患である。本疾患では，脳虚血の徴候を呈することが文献的に多く報告されている^1〜3,5）。しかし，頭頸部領域の腫瘤のみの症状で発見されることもあり^1〜3,6），この場合には症状が軽度であっても破裂，血栓，塞栓などの重篤な合併症の可能性を念頭において精査を進めなくてはならない。今回われわれが経験した本疾患3症例（分岐部近くに発生した2例，副咽頭間隙に発生した1例）について文献的考察を加えて報告する。

Three patients presented with only mass lesion without signs of cerebral ischemia. In two cases, an aneurysm occured immediate above the carotid bifurcation, and the definite diagnosis was made by means of digital subtraction angiography (DSA). The other case had a giant aneurysm in the para-pharyngeal space, which was embolized successfully with platinum microcoils under DSA. We emphasize that care must be taken for severe complications such as embolization, thrombosis and rupture during the course of the disease.
