動眼神経麻痺を呈した副鼻腔炎の2症例

Two cases of sinusitis associated with oculomotor palsy

Ⅰ．はじめに

　蝶形骨洞は種々の脳神経が通過する海綿静脈洞や視神経と隣接しており，蝶形骨洞に病変を有する副鼻腔疾患では視神経，外転神経，動眼神経などの脳神経症状をきたすことがある^1,2）。そのため，副鼻腔真菌症や細菌性副鼻腔炎などの炎症性副鼻腔疾患では蝶形骨洞を経て海綿静脈洞に炎症が波及し，脳神経障害を呈することがあり，診断や治療の遅れから，症状の改善が得られない場合もある^3）。副鼻腔疾患によって生じる動眼神経麻痺は，通常，視神経障害に伴って起こることが多く^3），副鼻腔疾患による動眼神経単独の麻痺は稀である^4,5）。今回，われわれの施設で副鼻腔炎による一側の動眼神経麻痺のみを呈した症例に対して内視鏡下鼻内手術（ESS）を行い，症状の改善を認めた2例を経験したので報告する。

　Two patients with inflammatory paranasal sinus disease accompanied by only oculomotor disturbances were surgically treated at our institution in 2006 and 2007. Lesions were found in the sphenoidal sinuses in both patients. One had aspergilosis and candidasis. The other had only bacterial infection. The occulomotor disturbances improved after surgery in both patients.
