動眼神経麻痺および外転神経麻痺を呈した副鼻腔悪性リンパ腫の1例

A case of malignant lymphoma of the paranasal sinus with oculomotor and abducens nerve palsies

はじめに

　頭頸部領域における悪性リンパ腫のなかで，鼻副鼻腔原発のものは約18％とされる^1）。その中でも副鼻腔原発の頻度は低く，鼻副鼻腔悪性リンパ腫のうち約14％と報告されている^2）。組織型は鼻腔原発ではT細胞由来が多く，副鼻腔原発ではB細胞由来が多いとされるが^3），鼻副鼻腔悪性リンパ腫の検体は壊死組織が多いため病理診断を確定しづらく，診断に難渋することが少なくない。一方で，頰部痛や頰部腫脹などの症状や，CT上の軽度骨破壊所見，脳神経麻痺の有無が診断の一助となる可能性がある。今回，動眼神経麻痺および外転神経麻痺を呈し，副鼻腔炎急性増悪との鑑別を要した副鼻腔悪性リンパ腫の1例を経験したため，文献的考察を加え報告する。

　We present a case of paranasal sinus malignant lymphoma with oculomotor and abducens nerve palsies that required differentiation from acute exacerbation of sinusitis. A 92-year-old man was presented with complaints of right buccal pain and diplopia. Pansinusitis and right abducens nerve palsy of unknown cause were observed. Nasal symptoms improved after administration of antibacterial agents and steroids, but the diplopia persisted. However, a left oculomotor nerve palsy occurred subsequently. Endoscopic sinus surgery was performed, and the histopathologic examination was consistent with a diagnosis of diffuse large B-cell lymphoma. The patient was treated using R-CHOP and has been in remission.
