フォンレックリングハウゼン病に合併した鎖骨下動脈瘤の1症例

A case of subclavian arterial aneurysm with von Recklinghausen disease

I．はじめに

　耳鼻咽喉科・頭頸部外科領域では，日常診療において頸部腫脹や腫瘤はよく遭遇する病態である。原因は多岐に及ぶので，理学的所見や画像診断から診断が容易な典型的疾患から，診断に苦慮する症例までさまざまである。今回われわれは頸部腫脹を主訴とし，術中の血管造影で鎖骨下動脈瘤と診断した症例を経験したので報告する。

　A 49-year-old woman complained of sudden neck swelling. Physical examination showed a single, smooth swelling on the right side of the neck, extending from the supraclaviclar fossa into the medially behind the sternomastoid muscle. Angiography confirmed rupture of subclavian arterial aneurysm. Because it was impossible to stop bleeding by only embolizations, we removed hematoma and stitched up the vessel. Vascular complications in von Recklinghausen disease and the treatment for the vessels were discussed.
