中咽頭癌前壁型に対する舌骨上咽頭切開アプローチと舌弁後方移動術による再建を施行した1例

The Suprahyoid Approach for Carcinoma of the Tongue Base Combined with Set-Back Tongue Flap―A Report of Case―

I．はじめに

　中咽頭癌前壁型に対する手術経路として経口法，下顎離断法，経頸部法がある。再建材料は遊離皮弁や有形皮弁の遠隔皮弁が多く使用されている。

　今回われわれは，中咽頭癌前壁型（T3N1 M0）症例に対し，舌骨上咽頭切開アプローチで腫瘍を摘出し，舌弁後方移動術（set-back tongue flap）により再建した。本法は，舌動脈と舌下神経が温存可能な症例には顔面や下顎骨に切開を入れないので，機能的にも審美的にも優れ，手術時間や在院日数の短縮にも役立つものと考えられた。

　The key for the surgical treatment of carcinoma of the tongue base is to select the optimum approach to the tongue base without important dysfunction such as deglutition,speech and occlusion. Suprahyoid pharyngotomy was utilized for carcinoma of the tongue base（T3,N1,M0）,combined with the set-back tongue flap. This method was very useful for excellent exposure and rapid closure within minimum operating time and resulting in good function. Rapid healing and enhanced oral rehabilitation enabled the patient to leave the hospital in a short time. This method has ever been reported previously.
