Ejnell法を実施した両側声帯麻痺の3症例の検討

Lateral traction of the vocal cords with Ejnell's method in treatment of bilateral vocal cord paralysis

Ⅰ．はじめに

　両側声帯麻痺は呼吸困難や喘鳴をきたすことがあり，これまで声門開大術としてさまざまな外科的治療が工夫されてきた。Ejnellら^1）が報告した声帯外方移動術は甲状軟骨から針を刺入し，その針を通した牽引糸によって，声帯を外方に牽引固定するもので，組織侵襲が低くまた術後の音声成績も比較的良好であることから今日多くの施設で施行されている。

　今回，当科で経験した顔面骨骨折整復固定用のチタンプレートを使用した声帯外方移動術の3例について若干の文献的考察を加えて報告する。

□Abstract

　Bilateral vocal cord paralysis is a life-threatening condition. We performed Ejnell's operation in three patients suffering from dyspnea and stridor due to bilateral vocal cord palsy. In all the patients, dyspnea was improved and postoperative voice quality became socially acceptable. One patient had a slight problem in swallowing for one month postoperatively. In one patient, mobility of the unilateral vocal cord improved five months after the treatment, therefore the laterofixing thread was removed from the larynx. We concluded that this method is useful in the treatment of bilateral cord paralysis and regular follow-up is necessary for monitoring vocal cord movements.
