Nasogastric tube syndromeが疑われた両側声帯麻痺の1症例

A case of bilateral vocal folds immobility possibly caused by nasogastric tube syndrome

I．はじめに

　Nasogastric tube syndromeは経鼻胃管留置による合併症として比較的稀ではあるが，両側声帯麻痺を生じるため，時に致死的となる注意しなければならない疾患である。その病態は経鼻胃管によって直接後輪状被裂筋が圧迫され，感染および壊死を起こすことで両側の声帯麻痺をきたすとされている^1）。今回われわれは，突然両側声帯麻痺を発症し，その発症および経過からnasogastric tube syndromeが最も疑われた1症例を経験したので若干の文献考察を加えて報告する。

　A 74-year-old male who had heart failure and cerebral infarction,was referred to the department of otorhinolaryngology,concerning onset of stridor and dyspnea. Flexible laryngoscopy revealed bilateral vocal folds motion impairment（VFMI）and edematous left arytenoid compressed by nasogastric tube. MRI did not show any new cerebrovascular lesion,and no evidence,causing VFMI,such as head and neck tumor lesion,was detected. Since nasogastric tube syndrome was highly suspected,the nasogastric tube was removed,and administration of antibiotics and steroids was started. Two days after starting the treatment,laryngoscopy revealed recovering of vocal fold motion.
