深頸部膿瘍を合併した急性喉頭蓋炎の1例

A case of acute epiglottitis complicated with deep neck abscess

Ⅰ．はじめに

　頭頸部の感染症が頸部の間隙に波及して生じる深頸部膿瘍は縦隔洞炎や膿胸をきたし致死的となることもあり，外科的処置を要した症例が多く報告されている^1～9）。また，急性喉頭蓋炎は重症度によっては，時に上気道閉塞をきたし気道確保を必要とする。しかし，急性喉頭蓋炎から深頸部膿瘍を合併する症例は比較的稀である。

　今回われわれは，急性喉頭蓋炎を初感染巣とし，深頸部膿瘍を合併して外科的処置を要した1例を経験したので報告する。

　A 61-year-old male presented with sore throat and anterior cervical pain. Edema of the epiglottis and bilateral arytenoids was found. CT examination indicated abscess formation in the pre-epiglottic and pre-tracheal space. The patient immediately underwent tracheotomy and transcervical drainage. The postoperative course was uneventful,and edema of the epiglottis was improved.

　We considered that immediate cervical drainage was essential for the treatment of deep neck infection,especially abscess in the pre-tracheal or pre-epiglottic space.
