急性呼吸困難を生じたポリープ様声帯症例

Acute Dyspnea due to Polypoid Vocal Cords

I．はじめに

　ポリープ様声帯とは，声帯膜様部の全長にわたりび漫性に浮腫を生じるものとされている^1）。40歳以上の喫煙者に多く，その臨床症状は嗄声，咳，痰，咽喉頭異常感などが主である。呼吸困難は比較的少ないとされているが，米川ら^1）は149例中9例に呼吸困難を認めたと報告している。ただし，急性呼吸困難により緊急気管切開が必要になった症例は極めて稀で，われわれが渉猟し得た範囲では4例の報告を認めるのみである^2～5）。

　今回われわれは，ポリープ様声帯により急性呼吸困難をきたした症例を経験したので，文献的考察を加えて報告する。

　A 63-year-old female with smoking and drinking history,suffered from dyspnea after vocal abuse and visited the emergency room of our hospital. Tracheotomy under local anesthesia was performed immediately and laryngomicrosurgery was done under general anaesthesia. We considered that mechanical irritation of the vocal cords stimulated a retention cyst. And topical inflammation and sudden enlargement of the cyst was responsible for her dyspnea in addition to a diffuse edematous swelling of the both vocal cords.
