真珠腫性中耳炎の術後に合併した脳膿瘍

A Case of Intracranial Abscess after Cholesteatoma Operation

I．はじめに

　耳疾患が原因となる頭蓋内合併症は，抗生物質が発達し，CTなどの画像診断が普及した今日では稀と考えられている。しかし，いったん合併すればその症状は重篤で，見逃すことは許されず，常に念頭に置くべきである。これまで耳性頭蓋内合併症の報告は決して少なくないが，そのほとんどは中耳炎が直接波及した場合である。

　今回，真珠腫性中耳炎の手術後に発生した脳膿瘍の1例を経験した。この症例は真珠腫性中耳炎の術前・術中に直接波及している所見を有さず，脳膿瘍の発症が遅発性であったことから，その発生機序，感染経路を中心に文献的考察を行った。

　A 32-year-old male was admitted to our hospital because of left otorrhoea due to cholesteatoma,and underwent typeⅢtympanoplasty. Though the intracranial extention was not observed,intracranial abscess was complicated 14 days after the initial operation. Because CT showed a cerebral abscess in the tempral lobe,surgical drainage was performed by a neurosurgeon. Six years later,this patient is doing well without neurologic sequelae.
