耳下腺原発基底細胞腺癌の1例

A case of basal cell adenocarcinoma of the parotid gland

I．はじめに

　基底細胞腺癌は組織学的には低悪性度の腫瘍とされ，予後は比較良好であるが術前診断が困難で，適切な治療を選択しにくい腫瘍の1つである。全唾液腺悪性腫瘍のうち基底細胞腺癌の発生頻度は約0.6％と低く^1），本邦における耳下腺原発基底細胞腺癌の報告例は5例^2～6）にすぎない（表1）。今回われわれは，耳下腺に発生した基底細胞腺癌の1例を治療する機会を得たので，若干の文献的考察を含め報告する。

A 74-year-old woman was complaining of a swelling of the left parotid region. CT and MRI revealed a tumor in the superficial lobe of the left parotid gland,1cm in diameter with clear margin. Superficial lobectomy of the left parotid gland was performed and the tumor was diagnosed as basal cell adenocarcinoma. Immunohistochemical stainings for Ki-67 and p53 were found effective for the differential diagnosis.
