内境界膜下血腫の除去

Removal of hematoma beneath the internal limiting membrane of the retina

（V−23）　黄斑部を大きく占拠する内境界膜下血腫10例に内境界膜下血腫除去手術を施行した。手術は，硝子体切除を行ったのち，血腫上の内境界膜を剥離して血腫を吸引，除去した。10例中2例で黄斑部に円孔を認め，SF₆ガスタンポナーデを施行した。また，2例で除去した膜を電顕で観察し，内境界膜であることを確認した。血腫の原因は9例が網膜細動脈瘤，1例は不明であったが，全例で合併症もなく視力の改善がみられた。内境界膜下血腫は自然吸収傾向もみられるが，症例によっては内境界膜下血腫除去手術が有効であると考えられた。

We surgically removed the hematoma beneath the internal limiting membrane involving the macula in 10 eyes. After core vitrectomy, the internal limiting membrane was peeled off and the hematoma was aspirated. A macular hole was found intraoperatively in 2 eyes, which received additional gas tamponade with sulfur hexafluoride. Morphological features typical for internal limiting membrane were confirmed in specimens from 2 eyes by electron microscopy. Retinal arteriolar macroaneurysm was the cause of hematoma in 9 eyes. Postoperatively, the visual acuity improved in all the 10 eyes with no significant complications. These cases illustrate that surgical removal may induce rapid and definitive improvement for hematoma beneath the internal limiting membrane.
