We performed vitreous surgery on 6 eyes of 3 patients of Terson syndrome with premacular hemorrhage. All were males aged 11months,22 years and 47 years respectively. The interval between onset and surgery was 4 weeks,5 weeks and 2months respectively. Dome-shaped premacular hematoma was present following removal of vitreous hemorrhage. The hematoma was aspirated through an opening in the membrane anterior to it using a soft-tipped back-flush needle. The peeled membrane was identified as internal limiting membrane(ILM)by electron microscopy. This findings shows that the dome-shaped premacular hematoma is located posterior to ILM in the present series of Terson syndrome. When the hemorrhage is located anterior to the macula in this syndrome,removal of hemorrhage through an opening in the membrane can led to early recovery of vision and can improve the patients'quality of life.
