抗リン脂質抗体症候群に伴う球後視神経炎の1例

Retrobulbar neuritis in a case of antiphospholipid antibody syndrome

（G−8PM−3）　31歳女性の右眼に視力低下が5日前に突発した。4年前に抗リン脂質抗体症候群と診断されている。右眼視力は手動弁で，中心暗点があった。CTとMRI検査で右視神経の腫脹があり，蛍光眼底造影で造影遅延などの異常がなく，球後視神経炎と診断した。ステロイドパルス療法開始5日後に，視力は1.2に回復した。血液の抗核抗体が陽性であり，ステロイド薬が奏効したことから，自己免疫性の球後視神経炎であると推定した。

A 31-year-old female presented with acute blurring in her right eye since 5 days before. She had been diagnosed as antiphospholipid antibody syndrome 4 years before. Her right visual acuity was hand motion. Perimetry showed central scotoma and peripheral contraction. Funduscopic and fluorescein angiographic findings were uneventful. Laboratory studies showed positive antinuclear antibody. Computerized tomography and magnetic resonance imaging showed swelling of right optic nerve, leading to the diagnosis of retrobulbar neuritis. Steroid pulse therapy induced recovery of right visual acuity 5 days later. Above findings seemed to suggest autoimmune retrobulbar neuritis.
