A 31-year-old female presented with acute blurring in her right eye since 5 days before. She had been diagnosed as antiphospholipid antibody syndrome 4 years before. Her right visual acuity was hand motion. Perimetry showed central scotoma and peripheral contraction. Funduscopic and fluorescein angiographic findings were uneventful. Laboratory studies showed positive antinuclear antibody. Computerized tomography and magnetic resonance imaging showed swelling of right optic nerve, leading to the diagnosis of retrobulbar neuritis. Steroid pulse therapy induced recovery of right visual acuity 5 days later. Above findings seemed to suggest autoimmune retrobulbar neuritis.
