虹彩炎が併発した角膜辺縁変性症（Terrien）の1例

A case of Terrien's marginal corneal degeneration associated by iritis

　虹彩炎が併発した両眼性のTerrien'smarginal corneal degenerationの1例を経験した。症例は66歳の女性で，主訴は両眼の視力低下であった。右角膜周辺部には混濁，菲薄化，表層血管侵入があり，中央部は高度な肥厚，混濁を示していた。菲薄化は角膜周辺部全周にわたり，いわゆるcontact lens cornea様であった。左角膜にも鼻側周辺部の菲薄化があり，中央部は肥厚，混濁を示していた。さらに両眼に漿液性虹彩炎の併発が認められた。治療による虹彩炎の消退とともに角膜中央部の肥厚，混濁は軽快し視力の向上をみたが，経過中，左角膜菲薄部が穿孔に至った。このため角膜上皮形成術（keratoepithelioplasty）を施行した。術後の経過は良好であった。

A 66-year-old female was referred to us on account of bilateral visual impairment of recent onset. The right cornea manifested circumferential furrow, marginal opacification and superficial vas-cularization inner to the limbus. The central cornea showed stromal edema and opacification simulat-ing contact lens cornea. The left cornea showed furrow and opacification only along the nasal lim-bus. Signs of serous were initially present but sub-sided following topical corticosteroid therapy. Two months after discontinuation of steroid, spontane-ous corneal perforation developed in the left cor-nea, necessitating immediate keratoepithelioplasty.
