We injected urokinase into the anterior chamber to treat postvitrectomy fibrin formation or blood clot there. The series included proliferative vitreo-retinopathy (PVR) 10 eyes and diabetic retinopath-y with or without PVD 5 eyes. All the eyes under-went pars plana vitrectomy and lensectomy. Pos-toperative fibrin formation in the anterior chamber developed in 14 eyes and blood clot in 1 eye. The amount of urokinase ranged 300 international unit (IU) 11 eyes, 600 IU 3 eyes and 1800 IU 1 eye. Urokinase was injected 3 to 15 days, average 5.9 days, after surgery. Fibrin formation resolved com-pletely within 3 hours in all the eyes. Blood clot became dispersed after 12 hours.

We injected 1200 IU of urokinase into the ante-rior chamber one day after vitrectomy and len-sectomy in 10 pigmented rabbit eyes. Light micros-copy showed absence of pathological changes in the cornea or the retina.

Above findings seemed to indicate the clinical safety and effectiveness of urokinase in the man-agement of fibrin formation and blood clot after vitrectomy and lensectomy.
