糖尿病性網膜症に対する汎網膜光凝固術後の長期経過

Longterm outcome of panretinal photocoagulation for diabetic retinopathy

　札幌医大眼科で汎網膜光凝固術（PRP）を受けた前増殖型および増殖型糖尿病性網膜症患者の長期観察例65例95眼について，その治療効果および問題点について検討した。

　視力の経過は，PRP終了後平均4.6年で改善13％，不変38％，悪化49％で半数は視力を維持することができた。視力低下の最大の原因は黄斑症によるもの（64％）であった。全体の84％に網膜症の改善が認められ，PRP終了から平均3.3年後には45％が停止性網膜症となった。PRP終了後に認められた黄斑部浮腫および硝子体出血はそれぞれ26％と41％であった。しかし，硝子体出血を起こした症例のうら67％は3.4年以内に自然吸収された。

We reviewed the clinical record of 95 eyes in 65 patients treated by panretinal photocoagulation (PRP) for preproliferative or proliferative diabetic retinopathy. After a mean of 4.6 years following treatment, the visual acuity improved in 13%, stayed the same in 38% and deteriorated in 49%. Progression of maculopathy was the major cause of decrease in vision in 64% of the affected group. Funduscopic features of retinopathy improved in 84%. In 45% of the 95 eyes, retinopathy turned into burned-out one after a mean of 3.3 years after treatment. Macular edema developed in 26% after PRP. Vitreous hemorrhage occurred in 41%, in which spontaneous resorption ensued in 67% of affected eyes.
