涙液分泌減少症における免疫学的考察

Immunological features of decreased tear secretion

　何らかの眼乾燥症状を訴えて受診した涙液分泌減少症患者72症例を対象に，免疫血清学的検査を施行した。全体で67％，男性に46％，女性に70％が自己免疫異常値を示した。フルオレセイン角膜染色とローズベンガル染色の結果より，臨床検査所見の悪化に伴い，免疫血清学的検査結果の陽性率も高まる傾向があった。涙液分泌減少症患者を診察した場合，基礎疾患として膠原病を含めた自己免疫疾患も念頭におくことが肝要であると思われた。

We evaluated 72 patients with moderate dry eyeregarding autoimmune antibodies. We also testedas to immunofluorescent antinuclear antibody,Sjögren's syndrome antibodies A and B, and RAlevel. The patient was categorized as autoimmunepositive when either of the above proved to be abnormal. A total of 48 patients, 67%, were autoim-mune positive. The incidence of positive findingwas 46% for males and 70% for females. Clinically,rose bengal score was most closely correlated withthe autoimmune positive status.
