We evaluated tear clearance and corneal sensitiv-ity as well as conjunctival sensitivity in order tostudy the tear fluid dynamics in diabetes mellitus.

In our study, the mean tear clearance rate in thediabetic group was 8.0+13.6-5.0, compared to16.0+5.1-3.9 in the control group (p<0.01). Themean corneal sensitivity (4.34±4.06 g/mm2) andconjunctival sensitivity (10.13+5.13 g/mm2) weresignificantly reduced in the diabetic group compar-ed to the controls (corneal sensitivity;0.97±0.08g/mm2, conjunctival sensitivity;4.77±1.63 g/mm2) (p<0.01). In Schirmer test,66% of the dryeye patients with diabetes mellitus showed under 5mm. There were correlations between corneal sen-sitivity and tear clearance rate (r=-0.56) (p< O.01), as well as between conjunctival sensitivity andtear clearance rate (r= -0.65) (p<0.01).

Tear dynamics in diabetics is suspected to reducecorneal sensitivity as well as reduce conjunctivalsensitivity which influence tear clearance rate. Thisfact suggests that it influences corneal fragibility indiabetics.
