小児2例にみられた低眼圧緑内障

Low tension glaucoma in two children

　低眼圧緑内障の臨床所見を示した小児2例を報告した。症例1は15歳男子で，頭痛の精査の際に眼底異常に気付いた。症例2は8歳女児で自覚症状はなく，偶然に眼底の異常に気付いた。両者の両眼の眼圧は日内変動を含めて正常範囲内であり，隅角は広隅角であった。視神経乳頭は緑内障性陥凹を示し，視野検査において，緑内障性変化を示した。CTあるいはMRI検査にて頭蓋内病変は認めなかった。低眼圧緑内障は高齢者に多い疾患であるが，若年者あるいは小児にも存在し，その発症メカニズムには加齢以外の要素も考えられる。

We observed low tension glaucoma in a 15-year-old boy and an 8-year-old girl. Headache led to detection of visual field defect in the first case and casual eye examination led to detection of glaucomatous disc cupping in the second. In both cases, the intraocular pressure was consistentlyunder 21mmHg with wide open chamber angle, glaucomatous disc cupping and field changes in both eyes. No intracranial lesions were detected by magnetic resonance imaging (MRI) or CT scan. The present cases show that low tension glaucoma may occur in children and that aging may not be a major etiological factor in this condition.
