フィブリン糊製コンタクト装用が奏効した偽膜型Stevens-Johnson症候群の1例

Contact lens of fibrinous paste was effective in a case of pseudomembranous Stevens-Johnson syndrome

　28歳女性に重篤な偽膜型Stevens-John-son症候群が発症した。連日瞼球癒着を鈍的に剥離し，粘弾性物質とステロイド眼軟膏を点入したが，癒着剥離に伴う角膜上皮の損失が著しく，予後不良と考えられた。瞼裂と結膜嚢を完全に覆うフィブリン糊製コンタクトを作製し，まず片眼に装用したところ短期間で劇的な改善をみた。そこで改めて両眼に装用し，瞼球癒着，パンヌス形成などの後遺症を防止することができた。

A 28-year-old female developed severe Stevens-Johnson syndrome with pseudomembranous con-junctival affection. The ocular lesion progressed in spite of daily treatments with blunt separation of symblepharon and topical instillation of viscoelas-tic material and corticosteroid ointment. We cov-ered the cornea and the bulbar conjunctiva of one eye by a contact membrane made of fibrinous paste. This procedure resulted in dramatic improve-ment in ocular finings. A similar procedure for the other eye was also effective. The patient was free of symblepharon or pannus formation as late com-plications.
