網膜中心静脈の循環障害に伴い毛様網膜動脈の循環障害が確認された症例

A case of central retinal vein occlusion associated with insufficiency of cilioretinal artery

　71歳女性の右眼にみられた網膜中心静脈閉塞症（CRVO）と毛様網膜動脈の循環障害について報告した。経過中CRVOの循環障害とそれに続く循環の改善がみられたが，それに対応して毛様網膜動脈にも循環障害と循環の改善が認められた。CRVOと毛様網膜動脈の循環障害の合併例においては，発病初期に毛様網膜動脈に血液の逆流現象がみられたり，急激な血圧低下により発症した症例がみられることなどから，毛様網膜動脈の循環障害は毛様網膜動脈と網膜静脈との灌流圧の低下に起因した機能的閉塞と考えられてきた。本症例は網膜静脈の循環状態と毛様網膜動脈の循環が密接に関連していることを示し，毛様網膜動脈の閉塞が機能的閉塞であることを裏付ける症例と考えられる。CRVOの循環障害時には毛様網膜動脈には口径不同，狭細化，充盈遅延が認められた。

A 71-year-old female presented with central retinal vein occlusion (CRVO) of venous stasis type in her right eye. Occlusion of cilioretinal artery was also present in the affected eye. As CRVO progres-sed during the following 3 weeks, choroid-retinal circulation lag also prolonged associated with narr-owing of the cilioretinal artery. When CRVO im-proved 7 weeks after onset, we observed animprovement in the impaired cilioretinal artery both by funduscopy and by fluorescein angiogra-phy. This case seemed to illustrate that cilioretinal artery insufficiency in CRVO is functional in nature, because the circulation in a cilioretinal artery is dependent on the perfusion pressure rela-tive to the central retinal vein.
