A 64-year-old male sought medical advice due to sudden impairment of vision in the right eye 2 weeks before. The funduscopic findings were compatible with the diagnosis of central retinal artery occlusion. We observed a bright plaque along an artery inferior to the macula. The plaque, which we presumed to be a cholesterol embolus, haddisappeared when seen 4 weeks later. There was a history of cardiac and cerebral infarction. Abnormalities of ophthalmodynamometry, arm-to-retina and arm-to-choroid circulation time were unilateral. Subsequent intravenous digital subtraction angiography led to the detection of occlusion of right internal carotid artery at the site of bifurcation.
