　コレステロール栓子は，網膜動脈を完全閉塞することは少なく，一過性黒内障の原因となったり，無症候性に動脈分岐部に光輝を放つ栓子として観察され，頸部内頸動脈閉塞症との関わりが深い。

　われわれは，64歳男性の右眼の網膜動脈を一旦閉塞したのち壊れて移動したと考えられるコレステロール栓子を経験した。右眼の眼底血圧の低下，腕・網膜および，腕・脈絡膜循環時間の遅延，右側脳梗塞の既往から，内頸動脈閉塞症が疑われ，静注digital subtraction angiography（DSA）検査で右頸部内頸動脈閉塞症が証明された。

　本症例は，近年本邦でも増加しつつある頸部内頸動脈閉塞症の急性眼病変の一つとして重要であり，診断確定における静注DSA検査の意義は大きいと考えられた。

A 64-year-old male sought medical advice due to sudden impairment of vision in the right eye 2 weeks before. The funduscopic findings were compatible with the diagnosis of central retinal artery occlusion. We observed a bright plaque along an artery inferior to the macula. The plaque, which we presumed to be a cholesterol embolus, haddisappeared when seen 4 weeks later. There was a history of cardiac and cerebral infarction. Abnormalities of ophthalmodynamometry, arm-to-retina and arm-to-choroid circulation time were unilateral. Subsequent intravenous digital subtraction angiography led to the detection of occlusion of right internal carotid artery at the site of bifurcation.
