糖尿病網膜症に合併した内頸動脈閉塞の2例

Two cases of diabetic retinopathy with carotid artery occlusion

（P3-2-30）　糖尿病網膜症に合併した内頸動脈閉塞症の2例を経験した。2例とも眼底所見に左右差があり，片眼に虹彩ルベオーシスと血管新生緑内障がみられた。MRアンジオグラフィで虹彩ルベオーシスのある眼と同側の内頸動脈閉塞が確認された。網膜光凝固を行ったが，視力予後は不良であった。眼底所見に左右差があり，片眼に虹彩ルベオーシスがみられる糖尿病網膜症では内頸動脈閉塞を疑って，侵襲のないMRアンジオグラフィなどを行って診断を確定する必要がある。

We observed two males, aged 60 and 70 years respectively, who presented with bilateral diabetic retinopathy and unilateral carotid artery occlusion. Both cases showed asymmetric fundus involvement with rubeosis iridis and neovascular glaucoma in the advanced eye. Magnetic resonance angiography showed ipsilateral occlusion of internal carotid artery in both cases. Both cases resulted in poor visual outcome in spite of retinal photocoagulation. These cases illustrate that carotid artery occlusion may be present in cases of diabetic retinopathy with unilateral rubeosis iridis.
